Stanlerina is Helen Lovejoy's cousin. She is transgender (male-to-female).

History

When the Simpson family discovered Grampa's storage locker with his professional wrestling regalia from the 1950s, they thought that Grampa was gay and had been in the closet for decades. They went to visit Grampa at Springfield Retirement Castle to tell him that they knew and that they loved him regardless. To express their support, Marge had Homer, Bart and Lisa wear T-shirts with gay-friendly symbols and messages, which prompted Bart to ask Marge if she had had them wear the shirts just because "it [was] cool to be accepting of a gay family member". Marge denied it, but then gave the lie to her denial as she grumbled about how Helen Lovejoy was "always bragging about her he-she cousin". (Later, the family learned the truth about Grampa's long-ago wrestling persona.)

After they left the Retirement Castle, they encountered Helen Lovejoy and Stanlerina on the sidewalk outside. Helen said she was "taking Stanlerina to get her hormone shots".
