"The Last Man Expanding" is the thirteenth episode of broadcast season 36 of The Simpsons and the seven hundred eighty-fourth episode overall. It originally aired on April 6, 2025. The episode was written by J. Stewart Burns and directed by Timothy Bailey. It guest stars Sidse Babett Knudsen as the Othinquic pen and Joe Mantegna as Fat Tony.

Synopsis[edit]

"When the people of Springfield discover the medication Othinquic has a weight loss side effect, everyone in town starts using it, except Homer. When Marge convinces Homer to take it, he finds that the drug has become impossible to find and turns to the Springfield Mafia to get it. However, as the only fat man left, Homer becomes a target for the townspeople."


Plot[edit]

At Moe's, everyone starts noticing something missing, then suddenly realizes it's Barney. A skinny man enters the bar looking like Barney. They realize it's him when he starts burping, and he says he has to take tests from his doctor and that he has hyperglycemia, which is why he has to take a medicine to regulate his glucose, Othinquic, and that the weight loss is a side effect. Another side effect is that his craving for alcohol disappears. Moe tries to hide the secret, but Chief Wiggum overhears it, and one week later, he easily manages to catch Snake. Seeing this, more Springfielders take notice and go to the doctor for the prescription.

The only one not agreeing to take the medicine, besides kids who are prohibited, is Homer. After arguing over his decision, he reserves a table at Luigi's. Marge is left alone with Maggie at home and starts hallucinating Othinquic talking to her in the form of a pen, showing her two possible futures: one where Homer takes the drug and has a six-pack, and one where he doesn't and he's dead, with two separate tombs. At night, Marge dreams of him exploding from inflating too much and wakes him up, talking him into taking it.

The next day, Marge is watching Opal on her show talking about the drug with Patty and Selma, who have taken the drug too but don't admit to it, and announce they're getting married to new fiancés, their co-workers. Homer is at the former hospital, now the Springfield Weight Loss Center, due to everyone not being sick anymore, and Dr. Hibbert mentions how it costs $900 per dose. When Homer points out that he's lying about losing weight from the drug, Hibbert kicks him out. Homer finds his way to a side road, where Fat Tony sells him the drug.

Everyone around Springfield becomes miserable, reverting to stealing, begging, walking around aimlessly, sad, and even Jebediah Springfield's statue is seen skinny. When Marge starts noticing the state of the people, after having a "talk" with the pen, she runs home, where Homer is about to take the drug. She stops him in time, but he gets kidnapped by the Springfield Mafia, who need him for liposuction as everyone becomes flaccid. However, Marge arrives at the scene to take him back, backed up by Luigi, Moe, and other chefs and food vendors.

In the end, Homer convinces everyone to go back to their food habits to regain what they lost—their happiness (and to lose the flaccid look they got)—with a speech, and all returns to normal. Patty and Selma invite Homer and Marge to their Sologamy celebration, but Marge hallucinates about the pen again, which asks her what they'll wear to attend. Marge wants to wear a dress that doesn't fit her anymore, so the pen convinces her to take the drug, making her blind, a 3% chance side effect. At the end, a montage of people regaining their weight is shown, including one where Mr. Burns is delivered to Putin.
