"The Last Man Expanding" is the sixteenth episode of season 36 of The Simpsons and the seven hundred eighty-fourth episode overall. It originally aired on April 6, 2025. The episode was written by J. Stewart Burns and directed by Timothy Bailey. It guest stars Joe Mantegna as Fat Tony.

Synopsis
- "Springfield becomes hooked on a miracle weight loss drug… all except Homer."
Plot
At Moe's, everyone start noticing something missing, when suddenly they realize it's Barney, when suddenly a skinny man enters the bar, looking like Barney. They realize it's him when he starts burping, and he said he had to take tests from his doctor and that he has hyerglycemia and that's why he had to take a medicine to regulate his glucose, Othinquic, and that the weight loss is a side effect. Another side effect is that his crave for alcohol disappeared. Moe tries to hide the secret but Wiggum overhears it and one week later, he easily manages to catch Snake, and seeing this, more Springfielders take notice and go to the doctor for the prescription needed.
The only one not agreeing to take the medicine, besides kids which are prohibited, is Homer, which after arguing over his decision, reserves a table at Luigi's. Marge is left alone with Maggie at home and starts hallucinating Othinquic talking to her in the form of a pen, showing her the two possible futures, one where Homer takes the drug and has a 6-pack, and one where he doesn't and he's dead, with two separated tombs. At night, Marge dreams of him exploding from inflating too much and wakes him up, talking him into taking it.
The next day, Marge is watching Opal on her show talking about the drug, with Patty and Selma, who have taken the drug too, but don't admit to, and announce they're getting married to new fiancés, co-workers. Homer is at the former hospital, now the Springfield Weight Loss Center, due to everyone not being sick anymore, and Dr. Hibbert mentions how it costs $900 pper dose. When Homer points out how he's lying about losing weight by the drug, Hibbert kicks him out, and Homer finds his way into a side road, where Fat Tony sells him the drug.
